The EFS takes about five minutes, requires no special equipment, and covers nine domains including cognition, functional independence, and polypharmacy. That breadth matters when you've got a patient who walks well but can't recall their medications. Grip and gait alone would miss them entirely.
The scoring is straightforward: out of 17 points, with clear cutoffs for mild, moderate, and severe frailty. The cognitive screen embedded in it's crude, sure. But it catches obvious issues before they derail a care plan. I've seen clinics burn twenty minutes on a full MoCA when a quick clock-drawing test would have flagged the same red flag. The EFS doesn't pretend to be a diagnostic instrument—it's a triage tool that tells you who needs deeper evaluation. That's the right job for a frailty score.

The Clinical Frailty Scale deserves a spot for when you have ninety seconds, not five minutes. It's a single judgment call from 1 to 9, and while it's subjective, the inter-rater reliability holds up surprisingly well in trained hands. Use it in the emergency department, use it pre-op, use it when the patient is too exhausted for a longer assessment. Just don't pretend it gives you granular data—it doesn't.
On the other end, the Frailty Index—the cumulative deficit model—remains the gold standard for research. It's robust because it's flexible: you pick thirty to seventy deficits, tally them, and get a ratio that tracks mortality impressively well. But that flexibility is also its curse.
Two clinics can build completely different indices and call both "the Frailty Index," which makes cross-site comparison messy. Unless you're running a study or have an informatics team to automate the data pulls, skip it for routine clinical work.
You don't have to do it all at once
The realistic bottom line is this: start narrow and expand later. We fixed this in one practice by rolling out the EFS only for patients over 75 with new onset functional decline—maybe three or four a day—then gradually expanding to all new geriatric consults after three months. The staff got comfortable, the workflow settled, and compliance stayed above eighty percent. What usually breaks first is enthusiasm, not the tool itself.
One caution: don't let the score become a checkbox. A number that isn't acted upon is just bureaucratic noise. Pair the EFS result with a single next step—a medication review, a home safety assessment, a physio referral—or you've added paperwork without adding care. That's the trap that discredits frailty screening faster than any psychometric weakness.
Data limitations? Acknowledge them. The evidence base for frailty tools skews toward older, hospitalized, or institutionalized populations. Community-dwelling adults, younger seniors, and diverse ethnic groups remain understudied. So use the EFS as a starting point, but adjust the threshold based on your clinic's population. If you're seeing mostly independent 70-year-olds, you'll need different cutoffs than a nursing home consult service. The right answer is rarely a single tool—it's a tool used with judgment, revised as your data accumulates. Pick EFS for balance, keep CFS for speed, and build your audit trail as you go.
